PAYMENT

I accept payment by cash, check, or credit card before or at the time of service.

Unless other arrangements have been made, I will include a form to obtain your credit or debit card information in your initial intake information to be uploaded in your secure client portal prior to your first appointment.

If you’re curious about my payment policies, or my office policies in general, please read about them in more detail in our Welcome Letter/Informed Consent before we begin work together. If you have questions or concerns about these policies, let's discuss them at the outset, so that there are no surprises as we go forward.

If you are using a credit card, my preferred method of accepting is via Simple Practice, a secure and confidential service designed for therapists.

INSURANCE

Currently I do not accept insurance. If your plan covers out-of-network mental health services here are some questions to ask your insurance company. Please call them first to verify deductibles and any other policies they may have. I can provide you with a monthly billing statement (often known as a “superbill”) to submit for reimbursement to your insurance company. Payment must be provided to me directly at the time of service.

I also have a few sliding scale spots available. Please contact me for more information on this.

CANCELLATIONS

We ask clients to give at least 24 hours notice when canceling appointments. Late cancellations and no-shows will be charged the full amount.

COVID & PUBLIC HEALTH

As public health is a core value of Mind the Gap Counseling, I take a firm stance regarding Covid safety and believe in taking necessary steps toward protecting ALL community members.

For the time being, all therapy appointments will be conducted via telehealth.

I appreciate your understanding.

NO SURPRISE ACT & GOOD FAITH ESTIMATE

The Consolidated Appropriations Act of 2021 was enacted on December 27, 2020, and contains many provisions to help protect consumers from surprise medical bills under what's known as the No Surprises Act. The protections took effect on January 1, 2022, and apply to all healthcare providers in the U.S.

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, healthcare providers need to give patients who don’t have insurance, or who are not using insurance, an estimate of the expected charges for medical services, which includes mental health care.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, which includes mental health care.

You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises/consumers or call our office at (503) 750-5888. You may also view a fact sheet by clicking this link.
